About Preferred Pediatrics

Appointments not cancelled with at least 24 hours notice and on same day missed appointments the following fees will be accessed:

• $50.00 for a physical or long appointment 
• $25.00 for a sick appointment
• $15.00 for a flu shot/flu mist appointment
• $10.00 for a nurse visit

School, Daycare and Sports Physical Forms:  There is a $10.00 administrative fee to complete these forms unless they are presented for completion at the time of your appointment.  Please note that it is the parent’s responsibility to bring appropriate forms.

All after hour calls that are triaged by a nurse will be charged a fee of $20.00 per call which is not covered by your insurance.

Medical Records:  Records are sent to other physicians at no cost to the patient.  If the patient would like copies there is a $10.00 administrative fee in addition to $.50 per page up to 50 pages and $.25 per page thereafter.  A medical record request form must be completed and signed in order for the records to be copied and/or sent.  Please allow 5 to 7 days for processing and understand that failure to submit the proper request form will cause a delay in your request.

If you would like an account summary of all paid monies on your account, there will be a charge of $10.00 per account, not to exceed $25.00 per family.  This request would need to be submitted to our billing office at 540.834.4608 and you should allow 48-72 hours for processing.
