We participate with most insurance companies. Please confirm with your insurance company prior to your appointment that we are an approved provider. For all HMO insurances our doctor's name must appear on the insurance card or we may have to reschedule your appointment. Co-payments must be paid at the time of your appointment. We accept cash, checks, and all major credit cards including VISA, Mastercard, American Express, and Discover. There will be a $10.00 administrative fee if copayment is not paid at the time of appointment.
